Установить частоту инфицирования ткани опухоли при анальном раке вирусом папилломы человека (ВПЧ), его типовое разнообразие, определить взаимосвязь вирусологического ответа (ВО) и показателей полного клинического ответа (ПКО), общей (ОВ) и бессобытийной выживаемости (БСВ).</p></sec><sec><title>Пациенты и методы</title><p>Пациенты и методы. Обследован 41 пациент: 36 женщин (57,1 ± 8,8 лет) и 5 мужчин (55 ± 9,5 лет) с плоскоклеточным анальным раком; IА–II – 31,7 %, III–IIIА – 24,4 %, IIIВ – 43,9 %. Пациентам проводилась конформная лучевая терапия на опухоль и лимфоколлектор на фоне химиотерапии (митомицин, капецитабин). Соскобы с поверхности опухоли отбирали до химиолучевой терапии, по ее завершении, через 3 и 6 мес. ДНК ВПЧ 14 типов выявляли методом полимеразной цепной реакции (ПЦР). ВО считали элиминацию ВПЧ в ткани остаточной опухоли. Сравнивали ВО с показателями ПКО, ОВ и БСВ. Статистическую обработку проводили с использованием программы STATISTICA 10.0.</p></sec><sec><title>Результаты</title><p>Результаты. ВПЧ-положительный статус подтвержден у 34 (82,9 %) пациентов, ВПЧ-отрицательный результат зафиксирован у 7 (17,1 %) человек. ВПЧ в образцах опухоли в виде моно- – 73,5 %, микст-­инфекции – 26,5 %. Чаще обнаружены 16 (82,4 %), 18 (17,6 %) и 31 (8,8 %) типы вируса. После химиолучевой терапии ВПЧ был установлен у 53,8 %, через 3 мес.– у 35,7 %, через 6 мес.– у 34,6 % пациентов. Пациенты с ВО достигали ПКО в 88,2 % случаев, без ВО – в 45,5 % (р = 0,0022). В группе с ВО 3‑летняя БСВ составила 88,2 %, без ВО – 27,3 % (р = 0,00048).</p></sec><sec><title>Заключение</title><p>Заключение. Исследование демонстрирует клинический потенциал выявления ДНК ВПЧ в ткани остаточной опухоли для оценки эффективности химиолучевой терапии при местнораспространенном анальном раке. Пациенты с наличием ВО чаще достигали ПКО и отличались более высокой 3‑летней БСВ. При оценке прогноза эффективности химиолучевой терапии целесообразно принимать во внимание ВО и проводить его оценку одновременно с оценкой ПКО, а при первичной диагностике включать в план обследования анализ ткани опухоли на ДНК ВПЧ широкого спектра типов.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Purpose of the study</title><p>Purpose of the study. To establish the causes of tumor tissue infection in anal cancer with human papillomavirus (HPV), its typical diversity, to determine the relationship between virological response (VR) and complete clinical response (CCR), overall survival (OS) and event-free survival (EFS).</p></sec><sec><title>Patients and methods</title><p>Patients and methods. A total of 41 patients were examined: 36 women (57.1 ± 8.8 years) and 5 men (55 ± 9.5 years) with squamous cell anal cancer; IA–II – 31.7 %, III–IIIA – 24.4 %, IIIB – 43.9 %. The patients underwent conformal radiation therapy to the tumor and lymph collector against the background of chemotherapy (mitomycin, capecitabine). Scrapings from the tumor surface were collected before chemoradiotherapy, upon its completion, after 3 and 6 months. HPV DNA of 14 types was detected by polymerase chain reaction PCR. VR was considered the elimination of HPV in the residual tumor tissue. VR was compared with the indicators of CCR, OS and EFS. Statistical processing was performed using the STATISTICA 10.0 program.</p></sec><sec><title>Results</title><p>Results. HPV-positive status was confirmed in 34 patients (82.9 %), whereas 7 patients (17.1 %) were HPV-negative. In 73.5 % of HPV+ tumor samples, the virus was in the form of mono-, in 26.5 % – mixed infection. The most frequently detected types of the virus were 16 (82.4 %), 18 (17.6 %) and 31 (8.8 %). Immediately after chemoradiotherapy, HPV+ were 53.8 %, after 3 months – 35.7 %, after 6 months – 34.6 % of patients. Patients with VR achieved CCR in 88.2 % of cases, without VR – 45.5 % (p = 0.0022). In the group with VR, 3‑year EFS was 88.2 %, without VR – 27.3 % (p = 0.00048).</p></sec><sec><title>Conclusion</title><p>Conclusion. The study demonstrates the clinical potential of HPV DNA detection in residual tumor tissue for assessing the effectiveness of chemoradiotherapy in locally advanced anal cancer. Patients with VR more often achieved PCR and had a higher 3‑year EFS.
