Оценить результаты лечения больных раком ободочной кишки III–IV стадий, проживающих в Южном федеральном округе.</p></sec><sec><title>Пациенты и методы</title><p>Пациенты и методы. Ретроспективно проанализированы истории болезни 200 больных раком ободочной кишки III–IV стадий, проходивших лечение и контрольное обследование в ФГБУ «НМИЦ онкологии» Минздрава России в период с 2019 по 2024 г. Произведена оценка общей выживаемости (ОВ) пациентов и ее зависимость от пола, стадии заболевания, локализации первичной опухоли, наличия мутаций генов KRAS, NRAS, BRAF, микросателлитной нестабильности опухоли, тактики противоопухолевой терапии.</p></sec><sec><title>Результаты</title><p>Результаты. Медиана контрольного наблюдения за пациентами с момента их обращения в ФГБУ «НМИЦ онкологии» Минздрава России составила 1,4 года, 5‑летняя ОВ – 31 % (медиана выживаемости – 3,4 года). При оценке 5‑летней ОВ в зависимости от распространенности злокачественного процесса кумулятивная доля выживших на III стадии составила 46 %, на IV стадии – 18 % (р = 0,00005). Согласно результатам нашего исследования, значимое влияние на выживаемость пациентов оказало наличие мутаций генов KRAS (3‑летняя ОВ при «диком типе» – 53 % против 28 % при наличии мутации) и BRAF (3‑летняя ОВ в случаях отсутствия мутации – 42,5 % против 18 % при ее наличии), наличие MSI (3‑летняя ОВ для MSI – 67 % против 42,5 % при MSS); для больных на IV стадии – вид назначенного таргетного препарата. Нами отмечено улучшение показателей выживаемости больных, в лечении которых применялась комбинация ингибиторов сосудистого эндотелиального фактора роста (VEGF) и рецептора эпидермального фактора роста (EGFR) в сравнении с применением только ингибиторов VEGF. Трехлетняя ОВ в группе комбинации EGFR и VEGF составила 66 % (медиана выживаемости – 4,1 года), в группе VEGF – 31 % (медиана выживаемости – 2,1 года).</p></sec><sec><title>Заключение</title><p>Заключение. Около половины случаев (47,6 %) рака ободочной кишки диагностируются на поздних стадиях, которые отличаются неблагоприятным клиническим прогнозом при наличии таких характеристик опухоли, как мутировавшие и микросателлитно стабильные фенотипы. Это обусловливает важность применения генетически и эпигенетически ориентированного подхода для поиска новых терапевтических мишеней и разработки более эффективных соединений и схем лечения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Purpose of the study</title><p>Purpose of the study. To evaluate treatment outcomes in patients with stage III–IV colon cancer residing in the Southern Federal District of the Russian Federation.</p></sec><sec><title>Patients and methods</title><p>Patients and methods. A retrospective analysis was performed of the medical records of 200 patients with stage III–IV colon cancer who underwent treatment and follow-up at the National Medical Research Centre for Oncology between 2019 and 2024. Overall survival (OS) was assessed, as well as its association with sex, disease stage, primary tumor location, KRAS, NRAS, and BRAF mutation status, tumor microsatellite instability (MSI) status, and antitumor treatment strategy.</p></sec><sec><title>Results</title><p>Results. The median follow-up from the time of presentation to the National Medical Research Centre for Oncology was 1.4 years. The 5‑year OS rate was 31 %, with a median overall survival of 3.4 years. The 5‑year OS rate was significantly associated with disease stage, reaching 46 % in patients with stage III disease and 18 % in those with stage IV disease (p = 0.00005). The presence of KRAS mutations (3‑year OS: 53 % in the wild-type group vs. 28 % in the mutant group), BRAF mutations (42.5 % vs. 18 %, respectively), and MSI status (67 % for MSI tumors vs. 42.5 % for microsatellite-stable [MSS] tumors) significantly influenced survival outcomes. Among patients with stage IV disease, survival was also associated with the type of targeted therapy administered. Improved survival was observed in patients receiving combined inhibition of vascular endothelial growth factor (VEGF) and epidermal growth factor receptor (EGFR) compared with VEGF inhibitor-based therapy alone. The 3‑year OS rate was 66 % (median OS, 4.1 years) in the combined EGFR/VEGF inhibition group versus 31 % (median OS, 2.1 years) in the VEGF inhibitor group.</p></sec><sec><title>Conclusion</title><p>Conclusion. Nearly half of all colon cancer cases (47.6 %) were diagnosed at advanced stages, which are associated with an unfavorable prognosis, particularly in tumors harboring pathogenic mutations and exhibiting a microsatellite-stable phenotype.
